The official Corporate Center listing identifies this location as a base for executive leadership and administrative departments. The corporate telephone number is not identified here as a dedicated HR, billing, or patient-complaints hotline.

Corporate correspondence: Confirm the recipient and department before mailing documents. Do not send payments, original medical records, or sensitive patient information to headquarters unless the responsible Banner team instructs you to do so.

A hospital care concern, a bill, an insurance appeal, and an employee benefits question require different contacts. Use the option that matches the issue.

Purpose Contact What to know
Corporate inquiries 602-747-4000 Phoenix Corporate Center; use specialized contacts for account or patient matters.
General patient assistance 833-252-5535 Customer Care Center; ask for help identifying the appropriate service.
Hospital and doctor-visit bills 888-264-2127 Billing questions, account balances, and payment assistance.
Urgent care bills 866-807-9776 Billing contact listed specifically for Banner Urgent Care.
Home care bills 888-562-7513 Home Care contact in Banner’s billing directory.
Patient portal technical support 855-355-6500 Patient Account login and technical assistance, not medical advice.
Hospital Patient Relations or Administration Find the hospital’s telephone number Ask the hospital operator for the team handling your concern.
Medical records Patient medical records instructions Select the correct facility and request process.
Employee benefits 833-849-9825 Benefits Resource Center for employee benefits questions, not patient insurance claims.

The billing numbers above come from Banner’s billing contact directory. Check the company name and contact instructions on your statement before making a payment.

For an immediate care or safety concern, tell the treating nurse or doctor. Banner’s patient safety guidance encourages patients to raise concerns with their care team promptly.

For an unresolved hospital experience, call the facility and ask for Patient Relations, Administration, or the person responsible for patient grievances. For an outpatient visit, ask the clinic manager which complaint process applies. These steps can help organize your request:

  1. Identify the location and visit. Have the facility name, department, visit date, and earlier case reference available.
  2. Describe the concern clearly. Explain what happened, what remains unresolved, and who you have already contacted.
  3. State the outcome requested. Ask for an explanation, communication with the care team, a records review, or another specific response.
  4. Use a private submission channel. Ask how to provide supporting records securely and what authorization is needed when acting for another patient.
  5. Request follow-up. Ask for the case reference, responsible department, formal grievance instructions when applicable, and a written response or expected update date.

A billing question may need a separate billing review even when it relates to the same visit. Keep both case references. The steps above are practical suggestions, not a guarantee of a refund or a particular investigation outcome.

Reporting Care or Safety Concerns Outside Banner

The Centers for Medicare & Medicaid Services directory identifies State Survey Agencies that investigate healthcare-facility complaints. Choose the state where the care occurred, not automatically Arizona because Banner’s headquarters is there.

You may contact the appropriate agency independently of Banner’s internal process. Medicare’s complaint guidance also distinguishes facility safety issues, professional-conduct concerns, and insurance appeals.

Questioning a Charge or Insurance Balance

Contact the billing team for the service involved. Ask for an itemized explanation and compare the statement with your insurer’s explanation of benefits, which summarizes how the plan processed the claim.

Identify the disputed service date, charge, payment, or adjustment. Ask whether a corrected claim or additional information is needed, and request confirmation of any approved change. Banner’s billing FAQs provide additional guidance about statements and insurance processing.

Hospital and doctor billing hours: Banner lists Monday–Thursday, 8 a.m.–7 p.m.; Friday, 8 a.m.–5 p.m.; and Saturday, 8 a.m.–noon, all Mountain Standard Time. Holiday hours may vary. Do not assume these hours apply to other departments.

Help Paying a Bill

Banner’s financial assistance page explains discounted and charity-care programs. Select the state where you received care and follow that application’s instructions. Eligibility depends on the applicable policy and your circumstances; having insurance does not automatically rule out assistance.

Ask about available payment arrangements and the documents needed for a financial-assistance review. Send applications to the destination shown on the form rather than assuming headquarters processes them. Use the official billing page to reach payment tools.

Receiving care at a Banner facility and being enrolled in a Banner insurance plan are different relationships. For a coverage denial, authorization question, or plan appeal, use the contact information on your insurance card and the instructions in the plan’s notice.

Banner’s Plans and Networks directory identifies its insurance programs. Members should use their specific plan’s resources, such as Banner Medicare Advantage appeals or Banner – University Family Care/ACC grievances.

A hospital complaint does not replace an insurance appeal. Follow the deadline and submission method in your plan documents, and ask about expedited review when the situation is time-sensitive.

Patient Account Login Help

The Banner Patient Account page lists technical support at 855-355-6500 or 480-684-6000, Monday–Friday, 8 a.m.–4:30 p.m. Mountain Standard Time.

For login problems, note the error message and whether it occurs in the app or a browser. Use the official password-reset tools. For access to someone else’s information, follow Banner’s family-access or representative-authorization process rather than sharing passwords.

Appointments and Messages to Your Provider

The patient account supports appointment scheduling for certain services and messages to providers. Contact the treating office for scheduling changes, referrals, or questions about your care. Corporate and technical-support staff are not substitutes for the clinical team.

Requesting or Correcting Medical Records

Start with Banner’s medical records instructions. The page links to the authorized online request process and explains in-person options through hospital Health Information Management Services or a provider’s office.

Have identification and any required representative documentation ready. Surgery centers, physical therapy, and imaging services have separate instructions on the same page. For an inaccurate record, use the listed request-to-amend process rather than asking billing support to change clinical information.

Banner’s privacy concerns page directs patients to their hospital’s Patient Relations or Administration team through the hospital operator. Use that route for concerns about how patient information was used or disclosed.

For language assistance or disability-related communication support, notify staff at registration. The Notice of Nondiscrimination lists 602-747-6880 for the Discrimination Coordinator. This is a purpose-specific grievance contact, not general billing or customer service. The notice also explains federal civil-rights complaint options.

Job seekers should use Banner Health Careers. Current employees can start with the Employee Resources directory, which identifies MyWorkday and the MyHR Resource Center for HR and payroll matters.

For employee benefits questions, Banner publishes the Benefits Resource Center number, 833-849-9825, available Monday–Friday, 7 a.m.–7 p.m. Mountain Standard Time.

Current and former employees should confirm the authorized employment-verification process through their HR resources or employment records. The corporate number is not verified as a dedicated HR or employment-verification line. Do not send personnel documents through a patient complaint form.

About Banner Health and Its Nonprofit Structure

Banner Health describes itself as a nonprofit health system, not a publicly traded hospital company. Its official organization overview identifies operations in Arizona, California, Colorado, Nebraska, Nevada, and Wyoming.

The system includes hospitals, outpatient care, specialty services, research, medical education, and health insurance programs. Its nonprofit structure does not mean every service is free; patients should ask about coverage, estimated charges, and financial assistance for their circumstances.

These independent guides cover other healthcare organizations and insurance assistance. They are not Banner Health support channels.

Banner Health Reviews and Complaints Guidance

A useful review describes a firsthand experience with a facility, appointment, bill, records request, or support interaction. Identify the general location, issue, contact route, response, and whether the matter was resolved without disclosing sensitive medical details.

Distinguish a healthcare-provider concern from an insurance-plan dispute. Individual reviews are not clinical assessments and do not establish the quality of care across every Banner facility.

Banner Health Billing Issue

September 25, 2024

Banner Health,Unable to contact billing. I was seen in August at a Banner outpatient clinic in Casper Wyoming. The claim submitted to my insurance was for an entirely different name than the provider I saw. I spent 45 minutes trying to contact billing to no avail. They give 8 automated selections and then placed me on hold for a representative, but they play a loop of music and never connect with a representative. I called the customer service number only to be told she is an operator and can transfer me to billing which would be a waste of my time since I had already spent 45 minutes trying to talk to someone. This is absolutely Unacceptable I have never had such a difficult time trying to contact billing with a simple question. There is no ability to leave a message, no email and no direct line to billing. The claim submitted for my date of service should list the provider I saw.
