Five Star Senior Living Corporate Office: Former Address and Current Community Contacts

Five Star Senior Living’s former corporate office was in Newton, Massachusetts. Its parent, AlerisLife, ceased operations and entered a wind-down, while communities transferred to new management. Residents and families should identify their community’s current operator before relying on an old Five Star telephone number.

What Happened to Five Star Senior Living?

The September 2025 transition announcement described AlerisLife’s planned asset sales and wind-down. DHC’s later quarterly report says AlerisLife had ceased operations by the end of 2025. It also confirms that DHC completed the transfer of its formerly Five Star-managed communities to seven other managers and terminated the former master management agreement.

This is a significant change from older directories describing Five Star as the current operator of a nationwide community network. A familiar building name, old brochure or saved corporate number may no longer identify the organization handling today’s staffing, resident billing or care questions. Use the specific community’s latest management notice and current contact information.

A change of manager does not, by itself, establish that a community closed or that residents had to move. The status of an individual property must be checked separately. The DHC transfer figures describe its portfolio; they should not be used as a complete list of every property historically associated with Five Star.

Five Star and Former Community Contact Routes

Reason for contacting Contact Purpose
Current resident or family concern The community’s current executive director or administrator Start locally for service concerns and ask for the present operator’s escalation contact.
Billing, deposit or account statement The current community business office; also check the issuer of the disputed bill Identify the service dates and whether the transaction occurred before or after a management transfer.
Records or privacy request The community’s designated records or privacy contact Ask who holds the relevant records and what authorization is required.
Independent assistance for older adults 800-677-1116 — call or text
Eldercare Locator
An Administration for Community Living service connecting older adults and families with local resources.
Long-term care complaint assistance federal Long-Term Care Ombudsman program overview Learn about state ombudsman programs serving residents of nursing homes, assisted living and other covered residential settings.
Former employee payroll or benefits The employer or administrator identified on employment and transition documents Use the appropriate employment period and legal employer, rather than assuming the current operator holds every historical record.

The historical corporate address is background information, not a verified current destination for resident payments, confidential records or time-sensitive correspondence. Ask the responsible organization to confirm the recipient and delivery method before sending documents. A public review on this website does not open a case with a community, operator or government agency.

How to Find the Current Operator of a Former Five Star Community

  1. Identify the exact property. Record the community’s full name, street address, city and state. Similar names can belong to different properties, and a community may keep its name after management changes.
  2. Check the latest written notice. Look for a management-transition letter, current resident statement, updated agreement or recent communication from the community. Note the operator’s legal name and the effective date.
  3. Confirm with the administrator. Ask who currently manages the property, which regional office handles escalation and where billing or records questions should be sent. Request written contact details when you need to follow up.
  4. Check the relevant state record when needed. For a licensed care setting, compare the property’s address and license information with the state agency’s current facility listing. The appropriate agency depends on the state and type of service.

Separate the property owner, management company and licensed care provider when asking questions. These can be different organizations. A real estate owner’s investor-relations office is not automatically the team responsible for a resident’s daily care, medication concern or account adjustment.

Care, Billing and Records Questions After a Management Change

For a current service concern, contact the administrator or staff responsible for the service involved. Give a factual timeline, the location and what follow-up you need. Ask who will review the matter and how the response will be communicated. For an immediate emergency, call 911 rather than waiting for a corporate email or public review to be read.

For a billing dispute, compare the dates of service, agreement, itemized statement and payment records. Identify the exact charge, credit or deposit in question. Ask whether the business office is reviewing a current-operator account or needs to direct you to a former operator’s records contact. A change of management alone does not answer who is responsible for a particular historical balance.

For a move-out or deposit question, keep the notice, move-out date, final statement and any written explanation of deductions together. Ask for an itemized response and the department handling the adjustment. Obtain current payment instructions privately; do not send money to an address solely because it appears in an old company directory.

For medical or resident records, ask who is the record custodian for the relevant period and what request process applies. State whether you are the resident or acting with authorization. The receiving organization may need to verify your authority before discussing records. Send sensitive documents through its designated secure method rather than posting them with a review.

For a complaint that remains unresolved, ask the current operator for the next level of review. Keep dates, case references and written responses. If independent help is needed, Eldercare Locator can connect you with local resources, and a state long-term care ombudsman may help with concerns in a covered residential setting. Ombudsman services and facility licensing oversight serve different purposes.

Five Star Corporate Office Questions

Is 400 Centre Street still the correct corporate address?

It should not be presented as the current headquarters. AlerisLife’s 2022 filing documents the move from 400 Centre Street to 255 Washington Street. Even the later address is now historical in this guide because of the subsequent cessation of operations and wind-down. Confirm a current recipient before mailing anything.

Is 617-796-8387 still the customer-service or HR number?

The number is documented in an earlier company filing, but an active dedicated resident-service, HR or payroll function was not verified for it. Use current community contacts for resident matters and the employer information on your employment documents for work-related questions. We do not label a historical switchboard as a current specialist help line.

Does a new operator automatically handle an old refund request?

Do not assume that it does. Give the business office the transaction date, former company name and relevant statement, then ask which organization is handling that matter. Keep the written explanation. This contact guide does not determine contractual responsibility or whether a particular payment is owed.

Can a long-term care ombudsman help an independent-living resident?

Ask the local program about the particular setting. The federal overview describes nursing homes, assisted living, board-and-care and other residential care communities, but coverage and available assistance depend on the setting and program. Eldercare Locator can help identify a local resource when you are unsure where to start.

Where should former employees request payroll or employment records?

Use the legal employer shown on the relevant pay statement and any contact provided during the transition. Include the employment dates and former work location when asking where to submit a request. A new operator’s payroll department may need to distinguish its own employment period from records held by a predecessor or benefits administrator.

For other senior living companies, see our Brookdale and Sunrise Senior Living guides. These are separate company resources, not a list of confirmed replacement operators for your community.

Why Trust Corporate Office Headquarters?

We checked AlerisLife’s historical corporate filing, the transition announcement, DHC’s current quarterly filing and federal elder-assistance resources on September 12, 2026. Phone numbers and contact details were checked against published sources, not by calling. Corporate Office Headquarters is independent and is not affiliated with Five Star Senior Living, AlerisLife or DHC. Practical preparation and escalation steps are our suggestions; the company’s own procedures and your transaction documents determine how a request is handled.
