Brookdale Corporate Office and Headquarters Contact Information

Brookdale Senior Living Inc. operates senior-living communities offering services that may include independent living, assisted living, memory care, skilled nursing, continuing-care retirement communities, and home-based services. Services, ownership arrangements, licenses, pricing, and available levels of care can vary by community.

Brookdale Corporate Office vs. a Local Community

Brookdale corporate headquarters handles company-wide leadership, investor relations, legal and regulatory matters, corporate careers, human resources, privacy, risk management, media relations, business development, and higher-level unresolved concerns.

A local Brookdale community is normally the first contact for matters involving a particular resident, apartment, room, care plan, meal, invoice, employee interaction, activity, transportation arrangement, maintenance request, move-in, move-out, or daily community operation. Local leadership may include an executive director, administrator, health and wellness director, business office manager, dining director, maintenance director, or another department leader.

Contacting headquarters first may delay a response when the issue requires access to local records or staff. Begin with the community unless the concern involves corporate policy, an unresolved escalation, a privacy matter, investor relations, corporate employment, or another company-level issue.

Choose the Correct Brookdale Support Route

  • Current resident or family concern: Contact the community’s executive director or administrator, then call Resident and Family Connection at 1-877-400-5296 if the matter remains unresolved.
  • New community, tour, pricing, or availability question: Contact a Brookdale Senior Living Advisor at 1-877-384-8989.
  • Monthly bill or payment question: Contact the local community business office or call Billing Support at 1-800-383-9993.
  • Job application or human-resources question: Call Brookdale HR at 1-844-899-6213 or use the official Brookdale Careers site.
  • Care-plan or service question: Contact the community’s appropriate care or wellness leader and request a private meeting.
  • Medication-related concern: Contact the community’s clinical leadership and the resident’s licensed healthcare professional through private channels.
  • Maintenance, housekeeping, dining, or transportation concern: Start with the department manager and community executive director.
  • Privacy concern: Use Brookdale’s official privacy contact route rather than posting medical, financial, or resident information publicly.
  • Investor question: Use Brookdale Investor Relations.
  • Media request: Use the Brookdale Newsroom contact route.
  • Unresolved company-level complaint: Contact Brookdale headquarters after documenting the local and departmental routes already attempted.

What to Have Before Contacting Brookdale Corporate Office

  • The Brookdale community name, city, and state
  • The resident’s name, shared only through an authorized private channel
  • Your relationship to the resident and authority to discuss the matter
  • The dates and approximate times connected to the concern
  • The names and positions of community representatives already contacted
  • A copy of the relevant agreement, invoice, statement, notice, or correspondence
  • A timeline of the issue and previous responses
  • Any case or reference number provided by Brookdale
  • The specific resolution or follow-up you are requesting

Do not publish resident names, diagnoses, prescriptions, care plans, medical records, account numbers, insurance information, Social Security numbers, payment information, employee personal information, home addresses, phone numbers, email addresses, or identifying photographs in a public comment.

How to Escalate a Brookdale Resident or Family Complaint

Begin with the employee or department directly responsible for the issue. When appropriate, ask to speak privately with the department manager, community executive director, or administrator. Explain what happened, when it occurred, who was involved, what steps have already been taken, and what resolution you are requesting.

If community leadership does not resolve the matter, call Brookdale’s Resident and Family Connection line at 1-877-400-5296. This route is intended for current residents and family members with comments, questions, suggestions, compliments, or concerns.

Ask for a case or reference number and keep notes showing the date, time, representative, promised next step, and expected response date. Follow up in writing when the concern involves billing, contractual terms, discharge or transfer notices, resident rights, care-plan changes, repeated service issues, or another matter where an accurate record is important.

If the issue remains unresolved after community leadership and Resident and Family Connection have reviewed it, send concise written correspondence to Brookdale Senior Living Inc., 105 Westwood Place, Suite 400, Brentwood, TN 37027. Send copies rather than original documents and remove unnecessary private health, financial, or identity information.

Urgent Safety, Abuse, Neglect, and Resident-Rights Concerns

Do not wait for a corporate-office response when a resident faces an immediate medical or safety emergency. Call 911 or the appropriate local emergency service and notify community leadership.

Concerns involving suspected abuse, neglect, exploitation, unsafe conditions, improper discharge, resident rights, or serious care failures may also be reported to the appropriate state licensing agency, adult protective services agency, law-enforcement agency, or Long-Term Care Ombudsman program. The correct agency depends on the community’s location, level of care, and licensing category.

The federal Eldercare Locator can help residents and families find local aging services and Long-Term Care Ombudsman resources. Its information line is 1-800-677-1116.

A corporate complaint does not replace emergency assistance, medical care, mandatory reporting, law enforcement, regulatory reporting, or legal advice.

Brookdale Billing, Payments, Deposits, and Move-Out Complaints

Billing concerns may involve monthly statements, rent, care-service charges, level-of-care changes, deposits, move-in fees, late charges, pharmacy or third-party services, refunds, credits, payment posting, move-out charges, or questions about a residency agreement.

Start with the business office at the community involved because local staff can review the resident ledger, agreement, service changes, payment history, and supporting records. Brookdale’s Billing Support Helpline is available at 1-800-383-9993 for billing questions.

Keep copies of the residency agreement, fee schedules, invoices, payment confirmations, move-in and move-out notices, and written explanations. Ask Brookdale to identify the agreement provision or service record connected to a disputed charge.

Do not post billing statements, resident account numbers, bank information, payment-card details, insurance records, or signed agreements publicly.

Brookdale Care Plans, Medication, and Health-Related Concerns

Care-related concerns may involve a resident’s service plan, changing needs, assistance with daily activities, memory care, nutrition, falls, mobility, transportation, medication coordination, communication with healthcare providers, or whether a community can continue meeting a resident’s needs.

Request a private meeting with the community’s appropriate clinical or care leader. Bring a written list of questions and ask how the concern will be documented, who is responsible for follow-up, and when the plan will be reviewed again.

CorporateOfficeHeadquarters.com cannot evaluate a resident’s condition, recommend treatment, interpret medical records, or determine whether a particular level of care is appropriate. Medical questions should be directed to qualified healthcare professionals who know the resident’s history and current needs.

Brookdale Phillippi Creek Sarasota Disappointing

November 18, 2024

Brookdale, moving my mom out of Brookdale Phillippi Creek Sarasota. I moved my mom into Brookdale and within a week she was very withdrawn and acting strange so I found out that they never ordered her Ativan and Lyrica that she has been on long term so she was going through withdrawal. Since then I have experienced many problems with laundry not being done, showers, dirty sheets, beds unmade and mom sleeping on her mattress pad for a week. Many of the staff members have little English and are unable to communicate with patients. I’m an RN and have been in the medical field for many years and have contemplated filing a complaint with AHCA. I’m moving my mom this week. I would hope that you let her out of her contract as of the 30th as the care is sub-optimal. You had a $2500.00 fee that I paid in May 2024. this fee should be adequate to cover the last 2 weeks. She was supposed to get her room cleaned two days ago and a girl just came in and wiped her dressers and left without sweeping and moping. My mom’s floors are filthy, her feet are black from walking around. Her bed is extra filthy since her floors are so dirty.

Brookdale Termination Issue

November 12, 2024

To Whom It May Concern, I worked as the weekend front desk Receptionist at Brookdale in Yakima, WA, and I believe I was unfairly terminated. Here’s what happened: In mid-September, I requested September 29th off, following the required steps by emailing my manager and asking a coworker to cover my shift. After no clear response from management, another colleague verbally confirmed they’d cover the shift, leading me to believe everything was handled. However, on October 1st, I received a message from my manager stating I was terminated for not showing up. Despite following up to clarify the situation and explain the coverage arrangement, I was told I hadn’t received approval. This resulted in my immediate dismissal without warning, written notice, or any opportunity to explain. I enjoyed working with Brookdale’s residents and was a loyal, dedicated employee. It is disappointing to lose this job in such an abrupt and, I feel, unjust manner. I request a review of my termination and a response from HR to address this matter fairly. Sincerely, A Disappointed Former Employee
