Patients and medical providers should send medical-record requests to the Concentra center where services were provided.

Concentra states that sending an ordinary patient or provider request directly to its outside records vendor can delay processing. Attorney requests follow a separate Datavant submission process.

Requests to Amend Medical Records

Concentra provides a separate form for requesting an amendment to clinical information in a medical record. Clerical errors should generally be addressed with the treating location.

A request to amend a record does not guarantee that the requested change will be accepted. Follow the written process and retain the response.

Concentra Disappointing Physical Therapy Experience

May 10, 2025

Concentra, I do not recommend going to Concentra for physical therapy. My ankle fracture had healed, but on my first visit for PT, George performed a Subtalar/Talocrural Distraction Manipulation that I believe was unnecessary. Since that procedure, I’ve been in constant pain. This has been a very frustrating and painful experience.
